Recently, I was looking through the ESM for 76, or datamining (though it's not really hard to do, you just need Fo76EDIT), and I noticed some interesting new records under the weapon section. One of them is a genuinely unique Combat Rifle in terms of legendary effects, although it's attached to the Shear Terror quest and thus I don't want to spoil it further than that.
Instead, I'm going to focus on the
six named legendary weapons that're connected to survival mode challenges.
First, we have a named .44 Revolver called
"Medical Malpractice", which has the Medic's (crits heal you and your group) affix, as well as two VATS affixes, one that increases accuracy and one that decreases VATS costs. It's connected to the weekly survival mode challenge of

"Deal Critical Hits to Enemies"
Next, we have the 50 Cal MG called
"The Action Hero", which is a Two-Shot legendary with 15 percent faster firing and reloading. It can be gained from the weekly survival mode challenge of

"Implements of Destruction", though I'm not sure what exactly the challenge entails.
After that, we have a unique Fatman called
"The Guarantee", which is Instigating, Crippling, and reloads faster. It can be gained from the weekly survival mode challenge

"Crush, Kill, Destroy".
The fourth weapon is a Lever-Action Rifle with a familiar name, the
"Sole Survivor". It has the Assassin, Hitman, and Steadfast legendary effects, and can be gained from the weekly survival mode challenge of

"Complete Events".
The fifth weapon is a M79 grenade launcher named the
"Crushing Blow", and it only has two effects, the Instigating and Crippling effects. It can be gained from the weekly survival mode challenge

"Kill Legendary Enemies". It only has two effects, compared to the 3 effect weapons above, though this is unreleased and potentially unfinished content so that may change.
Finally, we have our sixth legendary,
"The Quick Fix", which is a Junkie's Switchblade with faster swing speed and +1 endurance. You get it from the weekly survival mode challenge…

"Survive".
And that's all I could find. There's not any armor with the same set-up, but that might change. Personally, I think the idea of putting unique rewards in Survival Mode is a decent way to make it appealing to people who may otherwise never play it, and none of them are rewards you technically couldn't get as a random legendary so they're not "locked behind" the mode.
