I know that given work on Starfield and TESVI it will probably be ten years or so before before Bethesda releases Fallout 5, but on the (low low low) chance that someone from Bethesda reads this and is inspired, I'm making a post about my take on Fallout 5.
To date Bethesda Fallout games have taken place on the East Coast, and I'm comfortable with Bethesda following that trend. I propose that Fallout 5 be set in Philadelphia and New York City, and the wasteland in between, as it would fill in the gap between D.C. and Boston nicely. 2299-2300 would be a cool time for the game to take place.
The protagonist (a vault dweller?) could begin the game outside of Philadelphia, and for whatever reason must make their way to New York City. Both cities are confirmed to have been hit in Fallout 4, so the ruins should be pretty irradiated / devastated like D.C. was. But with more skyscrapers, especially in New York City.
There are plenty of opportunities for cool historical / revolutionary era America references and quests such as Congrees Hall and the Liberty Bell in Philadelphia, crossing the Delaware at Trenton, and the Statue of Liberty in New York City.
Other cool landmarks could be Princeton, West Point, Central Park, the Empire State Building, and whatever Fallout's version of the World Trade Center is. Subways should make a comeback as well.
In terms of mutants, there would obviously be ghouls. It would make sense for there to be super mutants as well, perhaps from the Capital Wasteland. However, I would like to see some more prevalent unique mutations, perhaps due to New Plague exposure. That would be interesting.
I'd like to see original factions too. It would make sense for the BOS to have some presence (in Philadelphia, New York City or both) but it shouldn't be a major presence like it was in Fallout 3 and Fallout 4. The Children of Atom and Abbey of the Road would be a cool addition as well. And, as a side note it would be hilarious to see a full fledged raider gang that developed from the Tunnel Snakes in Fallout 3.
The plot could revolve around tracking down a long forgotten doomsday weapon and preventing the world from being destroyed for a second and final time, or something like that.
In terms of gameplay – keep the food/weapons/armor crafting system, ditch the settlement building, bring back RPG elements like skills and meaningful choices, keep the gunplay. Ending slides like the ones in Fallout New Vegas would be great.
And for the love of God, don't mess with already established lore, don't introduce the new 50millionth variety of power armor, and don't write a half assed story / quests.
Bring back the R91 Assault Rifle and Plasma Caster. That's about all.
