Faker and Doublelift
As is news to nobody now, Doublelift is the most titled player in NA with 6 wins. Doublelift has won a regional title 5 consecutive seasons (S5-S9), equalling Faker's streak (S3-S7) – and before you all go for my throat how "Faker won nothing in S4 xd", OGN winter 2013-2014 is a part of season 4. Faker has won titles in 6 different seasons (all-time record), Doublelift won them in 5 different seasons (all-time runner up).
With yesterday's LCK victory, Mata is the first player in history to win a title with on 4 different teams (MVP Ozone, RNG, KT, SKT).
There are several players who won on 3 different teams: Pray (NJBS, ROX, KZ), Peanut (ROX, SKT, KZ), Deft (SSB, EDG, KT) and Doublelift (CLG, TSM, TL).
Winning from 2 different positions or on 2 different teams
With today's LEC final, G2's AD carry Perkz has the opportunity to become the
first second (thanks to
/u/trulymadlymaybe for reminding me) player in the history of League of Legends to win a regional title from 2 different playing positions (mid and ADC). The first player to do so is Yellowstar (ADC and support). Perkz's victory would also put him at the top of EU standings for most titles, tying him at 5 with sOAZ and Yellowstar. Only 5 players have more than 5 titles (Faker, Doublelift, Karsa, Maple and Swordart).
Meanwhile, G2's mid laner Caps and OG's support Mithy have the opportunity to become the first players to win EU title with 2 different organizations (FNC and G2 for Caps, G2 and OG for Mithy). With the win, Caps would enter the club of players who won 3 or more regional titles (44 players from major regions have done that since 2013).
Today's LEC final will be the first time in EU's history where organizations (at least in name) that played against each other in the final before meet again. G2 and OG have first played each in the final of 2016 EU LCS spring.
Rookie and Duke
If iG makes it to the final of LPL spring 2019, Rookie and Duke will have the opportunity to join the club of players who won titles in 2 different regions. This was previously done by Deft, Mata, Acorn, Imp (all KR+CN) Impact (KR+NA), Huni (EU+KR) and Karsa (TW+CN). As JDG's AD carry Imp is already in the final of LPL spring 2019, he can also join the club of players who won 3+ regional titles, and the club of players who won on 3 different teams (MVP, LGD, JDG).
edit: iG made the LPL spring 2019 final.
I'm drawing these stats from my memory and the collection which I keep updating on LoL Titles subreddit.
