I think that the next Expansion's big bad will not be Bolvar, but will be something that has been teasing for even longer.
This expansion will have us facing off against the Lich King, but not Bolvar. It will be a literal LICH King.

At the end of the fight with Kel'Thuzad in Wrath there was no phylactery on his body which is something required to fully destroy a Lich.
When we Kel'Thuzad died at the end of the raid fight he yells out the final line.
"Agghhhh! Do… not… rejoice! Your victory is a hollow one, for I shall return with powers beyond your imagining!"
taking this a step further there is also the fact that in World of Warcraft: Chronicle Volume 3 they added in a little info about his defeat stating.
Kel'Thuzad did not have his phylactery on his person during his second defeat. His current status remains unknown, but rumors spread that his soul was lost to the Shadowlands
So if we are going to be heading to the Shadowlands then it would be the perfect time for him to make his big return.
Back during a Cata lore Q&A they also made it a point to state that The Cult of the Dammed remained independent of Bolvar and his scourge.

In WoD one of the weirdest and most mysterious storylines they left unfinished was the events of Admiral Taylor's death at his Garrison. Everyone was killed and raised by a necromancer named Ephial. All we know about his motivation were this.
Ephial's motives for betraying Taylor lay in the promises of power, offered by someone or something he refers to as the 'dark one'.
There is also the way the story played out in the death knight order hall. Where the Lich takes actions without telling the player that make you more easy for the Lich King (Bolvar) to get into your mind.
While you’ve been busy with the Broken Shore, the resident lich of Archerus, Amal’thazad, has been working on a secret project with the Lich King. He has inscribed runes that will deepen your connection and make it easier for you to talk mentally to the Lich King.
So it wouldn't be a huge leap to say that the Death Knights were pawns of the liches who are serving a true master in the shadow(land)s.
There is also the fact that Bolvar's eyes have always been fiery orange and now in the new leaked image they are clearly blue, so it wouldn't even shock me if Bolvar's will was being controlled or affected by Kel'Thuzad.
